Abstract

OpenAlex English

Allergic reactions to macrolides are relatively uncommon (0.4%–3.0% of treatments), but reports have been increasing in parallel with widespread use. Both immediate and delayed reactions have been reported and are generally mild; severe reactions have only sporadically been described. The diagnosis of macrolide allergy rests on drug challenge in low-risk cases, because of the paucity of diagnostic methods with reliable positive and negative predictive values. Drug provocation is generally safe and effective, and most challenges are tolerated without reaction or only mild cutaneous symptoms.
