Is the Primary Obesity Surgery Endolumenal as Effectiveand Reliable as the Surgical Procedures?

Background: In cases of morbid obesity, treatment is generally applied as either a surgical or endoscopic approach. The number of Primary Obesity Surgery Endolumenal (POSE) procedures is increasing but the reliability and effectiveness is unclear as yet. The aim of this study was to present a series of cases that required revision surgery due to POSE failure and to reveal possible alternative surgeries. Materials and Methods: A retrospective comparison was made of the data of obese patients with POSE failure and conversion to surgical procedures between 2016 and 2018 in respect of operation, medical illness, and body mass index results. Results: The patients comprised 60% females, 40% males with a mean age of 43.4 ± 13.2 years and mean follow-up period of 11 ± 8.6 months. On average, patients lost 22.8 ± 9.2 kg, with an average excess weight loss of 48.6%. Conclusion: No firm conclusions can be drawn from such a small group. Although sleeve gastrectomy seems to be a safe procedure and should be considered as the first technique to be applied following POSE failure, it is possible to perform gastric bypass on patients with this endoscopic precursor.
