Adalimumab has been employed for an extended period as an efficacious and secure agent for inflammatory bowel and rheumatic diseases. ABP 501 is one of the biosimilars of adalimumab and has a similar safety profile to that of the original adalimumab. This report presents the first documented case of a 42-year-old female with Crohn’s disease and ankylosing spondylitis who developed hemotympanum following administration of adalimumab biosimilar (ABP 501), resulting in a blue ear drum. Following the discontinuation of adalimumab, the patient demonstrated recovery with the administration of oral (levofloxacin, dexketoprofen) and topical (ciprofloxacin, dexamethasone) therapy. This case suggests that adalimumab may be associated with the development of hemotympanum, which presents as a blue ear drum.
