Predictive value of the mNUTRIC score and survival analysis in critically-ill patients hospitalized in the intensive care unit: A prospective observational study

Patients who are critically ill and hospitalized in the intensive care unit (ICU) are at risk of mortality, the rate of which varies from 15 to 56%. Clinical deterioration can occur because of nutritional deficiency. Therefore, the effect of the modified nutrition risk in critically ill (mNUTRIC) score on patient clinical status was investigated in a limited number of studies. We aimed to establish the prognostic value of the mNUTRIC score in critically-ill patients in the ICU and to examine its potential risks with laboratory parameters. In this prospective observational study, 85 patients who were followed in the ICU for longer than 24 hours were included and their mNUTRIC scores were defined. The mNUTRIC scores of surviving and non-surviving patients were calculated and clinical conditions of patients with and without low nutritional scores were analyzed. The predictive value of 28-day mortality in these patients was performed with receiver operating characteristic (ROC) analysis and survival analysis was performed. 28-day mortality analysis revealed that mNUTRIC scores were lower in the survived group than non-survived group. Furthermore, mortality incidence, renal replacement therapies, and mechanical ventilation were more frequent in patients with mNUTRIC scores >5. The Youden index and ROC curves were used to confirm their use as an indicator of mortality incidence. For ROC analysis, the cutoff value was 6.5 with an AUC of 81%. For 28-day survival analysis, the ROC curve was higher in patients with low mNUTRIC scores. The mNUTRIC score is a simple, applicable, and useful scoring system that can be used not only to calculate nutritional risk, but also to determine the incidence of mortality and other clinical conditions.
